DR. JEFFREY CLEMENTS
Optometrist & Practice Owner
Dr. Clements is originally from La Crosse, WI. He attended Luther College in Decorah, IA where he received his undergraduate degree in Biology and was a letterman on the Norse football team.
He received his Doctorate of Optometry from the Illinois College of Optometry in Chicago, IL in 2009, while earning clinical honors.
He is a member of the American Optometric Association, including Sports Vision and Contact Lens Sections, the Wisconsin Optometric Association, and Madison Area Optometric Society. He was awarded Wisconsin's Young Optometrist of the Year in 2018. He lives in Sun Prairie with his wife and three children.

DR. LISA ZARWELL
Optometrist
Dr. Zarwell is originally from Pennsylvania where she attended Allegheny College in Meadville, PA for her undergraduate education. She went on to receive her Doctorate of Optometry in 1999 from Illinois College of Optometry in Chicago, IL, where she was also awarded the Pepsi-Cola Scholarship for Professional Promise.
She is a member of the American Optometric Association,
the Wisconsin Optometric Association, and the Madison Area Optometric Society. Dr. Zarwell practiced optometry in Missouri
and the Madison area for seven years prior to joining CVC in 2006.
She lives in Verona with her husband and three children.

DR. EDWARD KOUTNIK
Optometrist
Dr. Koutnik is originally from Wisconsin and comes from a family of dental professionals. While his interest in optometry may have been an unconventional path, it quickly became a true calling. With the support of his family, he discovered how life-changing it can be to help others see clearly and manage complex eye conditions.
He earned his Bachelor of Science degree in Biology from the University of Wisconsin and received his Doctorate of Optometry from the Illinois College of Optometry in Chicago, IL. He went on to complete a fellowship in Advanced, Medically Necessary Contact Lenses at the Illinois Eye & Ear Infirmary through the University of Illinois at Chicago.
Dr. Koutnik has a special interest in treating patients with dry eye disease and is passionate about helping individuals regain visual comfort and clarity through personalized care. His favorite part of practicing optometry is seeing the moment hope returns to a patient’s face—whether it’s finally finding answers to long-standing eye health concerns or seeing, for the first time, the details they never even knew they were missing.
He lives in the Madison area with his wife and children and enjoys staying connected to the community both professionally and personally.

DR. KALLI CHOLES
Optometrist
Dr. Choles is originally from Oregon, WI. She attended the University of Wisconsin-Madison where she received her undergraduate degree in Genetics and a certificate in Global Health. She then went on to earn her Doctorate of Optometry from Midwestern University-Chicago College of Optometry in 2025, graduating Summa Cum Laude. She is a member of the American Optometric Association and Wisconsin Optometric Association. Dr. Choles lives in Madison where she enjoys spending time with friends and family, playing with her dog and attending Badger basketball games with her family, whom have had season tickets for over 60 years.
